Haven't seen the chart yet, but here are the nuts and bolts. The devil is in the detail and lawyers won't be far behind!!

Fetal Heart Rate Threat Advisory System Under the algorithm proposed by Dr. Parer and Dr. Ikeda, a fetal heart rate pattern would be placed in one of the following categories, with these interpretations and suggested actions:

▸ Green. Normal. No fetal acidemia. Very low risk of evolving into a more serious pattern. Action: Normal labor and delivery techniques.

▸ Blue. No central fetal acidemia, but acidemia may develop. Low risk of evolving to a more serious pattern.

Action: Conservative therapies. Have operating room (OR) available and physician informed.

▸ Yellow. Intermittent reductions in oxygen that may result in fetal oxygen debt. Moderate risk to evolve to a more serious pattern. Action: Increase surveillance and use conservative therapies. Have physician at the bedside and OR available. Inform anesthetist and newborn resuscitator.

▸ Orange. Fetus is on the verge of decompensation, and heart rate pattern has a high risk to evolve to a more dangerous pattern. Action: Conservative therapies, and prepare for urgent delivery. Have physician at bedside and anesthetist present. OR and newborn resuscitator should be available immediately. Perform labor and delivery in OR if possible.

▸ Red. Actual or impending damaging fetal asphyxia. Action: Urgent delivery in OR with physician, anesthetist, and newborn resuscitator.
