IUD's post partum

I offer IUD's in the immediate post partum all the time. I usually tell mom to wait at least 4 weeks, but to come see me before she has intercourse the first time. Hatcher et al. and the WHO rate post partum IUD placement at >4 weeks as a risk category 1. I have no problem putting them in. I've only done about 500 IUDs, but I've not had a perforation (yet).

One trick that I like to, hopefully avoid perforation is to release the IUD as soon as I leave the cervical canal, so I'm pushing the blunted T into the fundus and not the sharp pointy tip of the IUD with introducer.
