I just saw a "tip" in a throwaway--make a small midline and then extend it to the right to avoid extension to the sphincter.

I've not done this yet.

I make midline episiotomies when I make them, which is infrequently.

I do tend to make them with forcep deliveries, but not with vacuum ones, because that "feels" right.
