I should think that you're more worried about malignancy here than infection. A nice, quick culdocentesis can get you fluid for cytology, analysis of the fluid (transudate, exudate, pus), or blood. Doesn't sound like infection anyway but if the fluid were purulent, a culture obtained that way would be good enough.
