Your right, I usually just see cases of DVT or P.E. where 2.0 is o.k.

It's amazing how INR can creap up to >5.0, about once every 5 years I get a call from the Emerg or ICU to see a women with a massive intrcrainal or G.I. bleed who has had some incidently vaginal bleeding.
