I recall reading an article on the subject once a long time ago and the article stated, that out of some 17,000+ births at the North Central Bronx Maternity Unit, the only case of serious (or fatal, I can't remember offhand) aspiration they had ever had was in a mom who hadn't eaten in 36 hours before she aspirated. Their policy was pretty liberal in allowing both fluids and solids during labor.
