Amy, I had a patient last year with giant perianal and perineal condyloma. She was 26 weeks when I saw her and used electrocautery (under spinal anesthesia) to excise the condyloma. I tried not to cauterized too close to the normal skin and she had much less pain than I feared she would have post operatively. She did not need any post operative TCA and delivered at term without problem.
