Young, mid 20s, one child and BMI of 28. I think you probably hit the nail on the head and that is to give her the options and make her make the decision.

Whoah!!!How old is she, how many children, what is BMI? Unless she is perimenopausal, no medical worry. After all you have progesterone present which many believe is preventative for endo ca. I would encourage tolerate it..If she is OCD, give her choice of estrogen, pills with IUD or after removal, remove and get preg and/or get sterilized. Make it the patient's problem, not yours. sAm PS High BMI's produce xs estrogen will spot, but need the progesterone to balance their xs estrogen.

possibly atrophic, consider 6 weeks of unopposed estrogen (ie premarin 0.625) see if the spotting stops. agf
