I've always seen twins delivered in the OR with a double set up if mom refuses scheduled c/section in case of cord prolapse, abruption, etc. In that case, informed consent in great detail is needed for a vag delivery.

But in this case, could you have given nitro or terb to relax the cervix and uterus, to immediately get the second baby out in light of severe fetal distress? No time for a crash c/section without the double set up really.

I assume from your description the second twin died?
