Have a 29 y/o G1P1, 2 weeks PP. Uneventful labor, but had a partially retained placenta which resulted in about 400 cc EBL and several hours later she became symptomatic with a vaginal hematoma. Only symptom was "not feeling well", no c/o pain. Not sure of the exact EBL with the hematoma (haven't read surgical report), but hgb bottomed out at 6.2 from 11 at 38 weeks. She received 2 units PRBC while in hospital. Lactogenesis has occurred and baby is gaining well. Patient developed mastitis Christmas eve, but ABX given and she is feeling well, although tired. I know that Sheehan's is rare (1:10,000) but also know symptoms can show up weeks to years after the occurrence. None of my research talked about the extent of blood loss that can result in Sheehan's. Does anyone on the list have experience with this and can fill me in? Thank you. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
