I'm sorry I appeared to have baited and run on the topic of this statement. The last thing I wanted to do was *debate* and am heartened to read there are several here who think this is a good place for discussion. Ef wrote: "Been discussed before – IMHO – hospital setting should be the rule.. some people still smoke even after reading the Surgeon General's warning…" To use your analogy, we should make cigarettes safer for those who smoke them. In turn we should make home/birth center birth safer for women who choose it by regulating midwives, not blocking them. If Gordon and others fear independent midwifery without safeguards for patient safety, why not encourage regulation and educational competency for nurse and non-nurse midwives? Midwives don't want "carte blanche" they want licenses to practice legally and safely, as do the families who seek their services.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
