Re: Gyn: persistent HCGFrom: Garry E. Siegel, M.D. (garrys@mindspring.com)Wed Nov 15 18:57:20 2006
Thanks to all. Recapping: OR due to blood in the abdomen--empty uterus on scan, NO curettage done. Blood and clots in belly, no tissue report generated. Now, HCG crept up from 100 the day after surgery (was 292 preop) to 224--but it took about 10 days -14 days to do so. I really feel that this is persistent trophoblast in the tube from an "incomplete" tubal abortion. Glen, I have a number of 15% of cases with persistent HCG after salpingostomy. In this case, I did NOTHING to a normal appearing tube (except that it had a clot on the end that rinsed off). I think methrotrexate, no scan, no curettage is the way to go here. Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
