Darryl: One of the things we weren't taught is that the Burch can widen the urethra and actually cause ISD. Your patients problem was common with Burch's prior to urodynamics. Then when they were studied after the fact, ISD was noted. So the mantra developed that if we were going to do a Burch, be sure they didn't have ISD before we did the Burch. Now, we do the studies and find that while they didn't have it before, gee, now they do. So I suspect an obturator sling will be needed. I would put her on your favored military antichlolinergic as Burch also favors causing DI as well.... The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
