Belated input on the Dermoid. I recently removed a Dermoid from a 500 # 20 year old. The Dermoid was visible on all four of the flatplates used to xray her abd. Contrary to popular opinion, Veterinary schools' equipment may not be used on humans.

The Dermoid filled the entire abd fom side walls to side walls, gutter to gutter and pelvis to rib cage. Had to peel the capsule from adjacent structure. No we did not try to save this ovary. We did put in a filter in the Vena cava prior to the procedure using the IVUS (intravenous ultrasound) gadget.

No Scopes, no Morcellators..just real surgery!!!!!!!!!!

Well, that's all predicated on taking the ovary out in toto. I guess you're writing this ovary off since the patient is 46. I've taken pretty impressive dermoids out by shelling them and reconstructing the ovary. I'm not sure I'd attempt that with anything but a laparotomy incision (even if it's relatively small).
