I'm sure there are loads of different questionnaires out there, but this one is from the following review article. Since this reports on the CARE trial published in the NEJM, it should be something that we can all make use of, right?

The only level I paper published recently concerning this question is in the abdominal approach. The Colpopexy and Urinary Reduction Efforts (CARE) trial is a prospective randomized study [8] designed to assess whether the addition of the Burch colposuspension to abdominal sacrocolpopexy in women without preoperative symptoms of SUI decreases postoperative SUI. The patients were evaluated in a blind-folded fashion 3 months after surgery. 23.8% of patients in the Burch group and 44.1% of the controls satisfied at least one criterion for SUI (P < 0.001). 'Bothersome' (defined as a response of 'moderately' or 'quite a bit' on the Pelvic Floor Distress Inventory Questionnaire) stress incontinence was also significantly less common in the Burch group (6.1 versus 24.5%, P < 0.001) [9**]. Enrollment was stopped after the first interim analysis because of the significantly decreased frequency of SUI in the Burch group. Ongoing follow-up of these patients will provide further long-term data.
