Regarding use of forceps/vacuum at Cesarean:

I was taught to use Simpsons backwards at C/S, and never really liked them.

I have used vacuums at C/S (the Kimi Omni is nifty) when the incision is tight, once the head fills the incision.

For getting the head out of the pelvis or into the incision, there is an instrument called a Murless vector, which is basically a hinged forceps blade with a handle.
