We basically can always get reimbursement for the modifier 22. When the charge is entered I automatically drop the claim to paper and attach the supporting documentation. With some payers who have only paid the allowable I have also found their reimbursement polcies and attached it as supporting documentation. We normally get 20% over the allowable, but sometimes it takes more than one try. If the payer suggests that the documentation does not support the use of modifier 22, then have the physician dictate a letter explaining the additional risks and complications and send the claim for medical review
