Re: prolonged PP feverFrom: Dean Huffman . (dean@thehuffpeople.net)Tue Aug 29 20:50:17 2006
.. Of interest, Dr. Paul B. Beeson, the person, who along with Petersdorf, did the original work on FUO's (fevers of undetermined origin) just died on August 14 this month. The paper, was published in "Medicine" in 1961. - - - - Date: Mon, 28 Aug 2006 11:43:37 -0500 From: "Atkinson, Samuel M" <ATKINSONS@ECU.EDU> Subject: Re: prolonged PP fever May I be old fashioned and ask what does her pelvic exam reveal? CT not a replacement for real exam. A uterine dressing forceps in the cervical os, opened up to allow the catsup out. Look for bubles in the catsup and do an anaerobic culture. sAm -----Original Message----- From: ob-gyn-l@obgyn.net [mailto:ob-gyn-l@obgyn.net] On Behalf Of Betsy Hyde Sent: Thursday, August 24, 2006 9:09 PM To: Multiple recipients of list OB-GYN-L Subject: prolonged PP fever G1P1 now s/p c/s for something or other in labor. Don't remember what, and don't think it is relevant to the presentation. It wasn't sepsis. Now 10 days post c/s and still spiking to 101.5. Blood cultures while spiking are neg. EKG neg. No hx travel to tropical climates. Post op course includes normal CT scan, WBC never >11.9, courses of triples (amp/gent/clinda and also amp/gent/metronidazole). Now on amp/gent/metronidazole/heparin x 36 hours and still spiking to 101.5. No localizing signs. Patient feels fine. Today's plans include MRI. Thoughts??? How long does it take for septic thrombophlebitis to defervese on heparin? Anything else we should consider? I raised the possibility of valvular disease and vegetative emboli, but cultures during spikes were neg, and EKG was neg as well. Drug fever??? (MFM is involved as well as the OBs in my practice ... this is not a midwifery management question...) Betsy Hyde CNM Branford, CT

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
