Well, as it turns out, the baby I delivered back in June was not my last. You know what Robert Burns said about the best-laid plans...... After a year of every-other-Friday outreach clinic and occasional half-days of residency precepting, the residency director asked me if i wanted to become their weekday hospitalist for L&D/postpartum/nursery. So now, from 7AM-noon, 5 days a week, I round on postpartum moms & newborns, teach residents how to do circs, evaluate outpatients, and supervise deliveries. Still keeping the promise I made to my family---no nights, no weekends, no holidays. So here's how my first week has gone so far: Monday---A Vietnamese G2P1 who I had followed in outreach clinic came in in active labor (is it coincidence that I had told her I would not be able to deliver her baby unless she came in before noon, and not until at least 8/21?) I stayed a little past noon because she was just about complete. Big ol' shoulder dystocia my first day back. She didn't understand enough English to get her up on all-4's, so when McRoberts and suprapubic pressure didn't work, the nurses rolled her onto her side and I delivered the posterior shoulder that way. Baby didn't move right arm much the first several minutes, but all is well now. Tuesday----G1P0 being induced for oligo and poor BPP. Starting 4th hour of Cervidil when I came on at 7. Not doing anything much. Still thick and closed. 10:30 still thick and closed. 11:15 sudden massive decels, briefly responsive to repositioning, IVF, O2, and scalp stim. Next ctx massive decels again. Terbutaline and prep for C-sec. She rolled by the desk on her way to OR at 11:23 while I was calling her resident and the neonatologist. OB was still on his way in at that time. Time of birth 11:38. I didn't get to see what happened in OR. Too swamped with other stuff on L&D. Wednesday---3 circs. Just getting ready to start the 3rd when the second started bleeding. AGNO3, 3 stitches, and gelfoam. The worst part was no one could find me a needle holder or soap to wash him off with. At home, kids are moving in and out, starting 3 different schools, and we are hosting a Korean foreign exchange student (a sudden impulsive decision we made 2 weeks ago). Life is crazy.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
