Endometrial Implant

35 yo Korean multip who presented with LLQ pain following C-Section x2 at another institution, waxing and waning with menstrual flow. Had been blown off by other MD's previous when studies found no mass. Entered her sub-q from the edge of her old Pfannenstiel and sure enough got a flow of old, dark appearing fluid when I tunneled up into the nodule. Pathology returned endometriosis. Removed as much of it as I could - it was part and parcel with the fascia to a degree - and cauterized as much as I could. She was delighted with the outcome and had pain relief for many months post. She then returned with renewed discomfort in the same local. The mass is 'back'. I put her on continuous OCP's after much convincing - she does not like to take pills! - and she's done fairly well, but returned today with an episode of more profound discomfort several days ago which has since resolved. She says that she did not miss any pills. She wants DEFINITIVE therapy, "like LASER"! We talked about changing OCP's or trying a course of Lupron. Any other thoughts?
