Re: Vertex to Breech in laborFrom: Henry Gregor (henrygregor@yahoo.com)Sun Jun 4 18:30:37 2006
Great story Denise and it sounds as though you have handled the rough edges of your backup's behavior with grace and good humor. I have not had any turns in labor, but did have one in prodromal labor. Pt presented as breech, with hx of desultory and irregular u/c's for the past day or so. Bedside sono confirmed breech and I asked our local MFM for an opinion re version ... on his exam a while later, the presentation was vertex....slings of outrageous fortune flew subtly by, with a few winks and nods re the earlier findings, and implications of less than profound clinical and sono skills. Having worked in a sono department as a medical student, and having had the transducer eighteen centimeters above the patient's umblilicus when measuring the head dimension, I felt comfortable just saying thanks for the consult. With ruptured membranes. G2P1 SROM hours earlier and on pit. Scalp electrode placed at 4cms due to an inability to keep FHR tracing. Loss of contact due to lots of maternal position changes. No epidural. Checked her again a couple of hours later. Found her to be 8cms. Felt horrible to find out she was breech and that I had missed it. MD on call, not our usual MD, did not do vaginal breech deliveries. Called me a couple of choice adjectives in front of the family. I already felt like a total idiot. His assistance in this was not needed. In the OR I was unable to take the electrode off the baby's scalp. Nothing would work. Never noticed that it was a little "shorter". MD again not pleased with the CNM. Another list of adjectives. I just cut the electrode off at the introitus. Baby was born. Electrode on the head. MD did not say anything. NICU team kept mentioning that the electrode was still on the head. FOB was laughing that it was still on the head. RN joked about it. MD got upset and said, "OK, now everyone can just be quiet about it." Anesthesiologist just winked at me. Denise, CNM So Cal Tired of spam? Yahoo! Mail has the best spam protection around

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
