I agree with Dr.Ronald E. Ainsworth, MD, FACOG about viral risk of Rhogam but to avoid any kind of medico-legal problems I suggest to be prudent and obtain a patient's written consensus as You can see from below: http://www.rhogamfyi.com/rh-incompatibility.html

RhoGAM side effects The RhoGAM treatment that has been produced since April of 2001 can be administered with very little risk to the patient and the fetus. The patient should be aware, nevertheless, that RhoGAM can cause: • Local inflammation (swelling, redness, and induration) at the injection site • Slight fever • Viral infection (Because RhoGAM is made from human plasma, there is a potential for the patient to develop a viral infection. However, the occurrence of infection is miniscule because of the intense screening of the plasma.) The treatment may also be ineffective; the patient may produce antibodies despite receiving the shot. The incidence of RhoGAM side effects is about one in 60,000 doses. RhoGAM with thimerosal RhoGAM produced prior to April of 2001 contained thimerosal, which added potential risks for the patient. According to the American Academy of Pediatrics' September 1999 policy statement, ethylmercury (a component in thimerosal) has been associated with neurotoxicity when taken in high doses. Developing fetuses are particularly sensitive to mercury. The neurotoxicity of mercury has the potential to affect the development of a child's nervous system. Thimerosal Thimerosal is a preservative used to keep contamination out of many medicines and vaccines. It was used in RhoGAM prior to April of 2001 and contains ethylmercury as its acting component. Ethylmercury, when used in appropriate dosages, has not been proven to have dangerous side effects, but because of recent studies on the health effects of methylmercury by the EPA (Environmental Protection Agency), the FDA (Food and Drug Administration) is approaching ethylmercury with caution. The case against thimerosal Exposure to methylmercury, a separate mercury compound, produces symptoms nearly identical to those of autism. In addition, it is known that infants are especially susceptible to mercury poisoning. Ethylmercury (the component in thimerosal) has not been shown to have side effects like those of methylmercury, but parents with children suffering from ADHD (Attention-Deficit/Hyperactivity Disorder) and DSI (Sensory Integration Dysfunction) are demanding further investigation. The RhoGAM treatment that is currently being produced does not contain thimerosal. Patients who have been raising concern received the treatment between 1968 and 2001. These individuals claim that the ADHD and DSI symptoms their children are presently exhibiting are rooted in their prenatal exposure to mercury. RhoGAM lawyers are presenting these cases and pushing for further investigation into ethylmercury.

To my knowledge, there has never been a case of any type of viral infection caused by RhoGam. I do not suggest or counsel patients that they are at risk of any infectious disease, i.e., HIV, hepatitis, etc., when they receive RhoGam.

Here in Italy we do RHogam protection in Rh negative women only after abortion ,spontaneous or induced , after amniocentesis or chorionic villous sampling, within 48 hours after birth of a Rh positive baby and now after vaginal bleeding during pregnancy or abdominal trauma. We do every two months during pregnancy indirect Coombs anti_D We dont ask presumed father's blood group (mater semper certa, pater nunquam=we know who is the mother, not who is the father) P.S.:remember it derives from human blood (HIV,EPATITIS AND SO ON) ,needs patient's written consensus ,cannot be used in Jeovah followers

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
