mid 40s woman 2 or 3 kids, tubes tied. US shows fibroids and what appears to be two cysts back to back on one ovary. Pretty benign appearance by US. No free fluid. Repeated in 6 weeks showing about the same with a cyst on the other ovary also. CA 125 is 103. Radiologists say it looks pretty benign and suggests follow up in 3 months if direct visualization is not done. Second radiologist says do MRI now if not doing direct vis. My gut says to look now. Would you 1. Send to Gyn Onc for surgical evaluation for maximal debulking in case it is CA? 2. Do the Lapscope yourself with the probability of it being fibroids and endometriosis as the cause of both the findings on US and the CA12 of 103 3. Do an MRI and if still fairly benign repeat some imaging at some future point (if so - what and when and labs)? 4. Something else?
