46 yr. old G1P1, in mid 2004 underwent uneventful LSH for symptomatic adenomyosis. About 6 months postop, developed inflammation and purulence in base of moderately deep umbilicus. Incision intact. Rxd with H202, and antibiotics. 3 months later same sx recurred, Rx'd as before . Had her seen by general surgeon ( concerned re: ? stitch abscess). Suggested revision if one more recurrence. This did occur, revision performed in October. Path: endometriosis! I have never seen endo in an umbilical trocar incision. The uterus was removed via a LLQ incision through a morcellator. At time of LSH, no pelvic/ abdominal endometriosis seen. J. Glenn Bradley MD
