I definately repair all 2nd degree laceratios. I would not even consider not repairing one.

I used to not repair small periurethrals unless they were bleeding, but many years ago I was called back to L&D after one did start bleeding, so now I repair anything but a very superficial laceration. In addition, while most small 1st degrees and periurethrals will heal and do fine without suturing, the patient is much more uncomfortable if they are sutured.
