Re: early discharge

In Ontario (Canada!), women who birth in hospital are discharged at 3 hours postpartum. Provided, of course, we have no concerns regarding bleeding, baby's well-being, etc. GBS positive mothers stay in for 24 hours if adequate (over 4 hours) GBS prophylaxis, 48 hours if not. Women who get an epidural stay in at least 12 hours. We wheel them down to the car ourselves. They know to page if they have concerns. They are seen at home 12-24 hours after the birth, day 2 is a phone check-in, days 3 and 5 or 6 are home visits, then usually in the office at 2, 4 and 6 weeks - all adapted to the mother's needs, some women need more visits, some need less. C-sections are often a 48 hr discharge, as the doctors know we will be seeing them at home. In the 11 years since legislation, we have had at our practice one readmission for a woman who started hemorrhaging when she got home after 3hr discharge - it resolved by the time she got back to the hospital. Most common reason for readmission in the first week is hyperbilirubinemia. Problems with breastfeeding of course are addressed at home. This follow-up is one of the big reasons women really appreciate our care.
