Vickie--

It is totally inappropriate for the obstetrician you work with to go out of town leaving you with (really) no back-up and a patient ready to develop one of several postpartum and post-op complications.

I don't care how much confidence the administration and the obstetrician have in you, you know that caring for this woman alone is outside of your boundry of safety and scope of practice. Call their attention to your protocols which I'm sure clearly state that you independently manage only low risk patients. Nurse-midwives collaborate in the care of high risk patients while the obstetrician would direct the care of a patient like this.

In our practice, the we CNMs might see the patient in morning rounds while seeing other patients; however the obstetrician would see her later that afternoon, direct her care, write orders, and consult appropriately. Don't let them talk you into taking responsibility for this patient.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
