"Ngozi" is the Zulu/Ndebele word for "Danger". I won't make any jokes about your name. South Africa, and indeed any country in Africa, is desperately short of specialists. It is not easy to arrange registration to practise in South Africa however, but for someone keen to practise there, there is no shortage of places one could get a job. Urogynae is not a big specialty as prolapse is uncommon in the local black people, but fistula surgery is a good skill to have. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
