A thirty year old female complains of "hot flashes" and her description is consistent with PM type flushes, ie, a sensation or rising warmth from the thorax going cephalad, followed by deaphoresis and a chill, occurring most nights a week and about three to four days per week. Presents with hx s/p BTL, with past hx of laparoscopic ablation of pelvic endometriotic implants, with subsequent laparoscopy without recurrent/persistent endo changes, cycles regulary, has a normal FSH and normal TSH and FWIW a very adequate premenopausal estradiol serum level. She takes quinine salts for restless leg syndrome, but the thermoregulation problems predate the quinine therapy.Had been given HRT and ocp's without symptom improvement. My inclination is endocrine referral, but that is distant, not easiy travelled to by the patient, and the proposed consultant actually has expressed little interest in the situation. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
