OB: HypertensionFrom: Garry E. Siegel, M.D. (garrys@mindspring.com)Wed Jul 20 16:05:32 2005
37 YO P1011 at 36w5d, excellent dates. She is a petite patient whose blood pressure in her first pregancy ran 90 to 110/60 to 70; during gyn visits between pregnancies, 110/80 times 3 readings. With her first pregnancy 2.5 years ago, she went up to around 140/90 at 38 weeks, normal lab, no protein, and was managed serially for a while. She was induced around 39-ish weeks, and ultimately arrested at 5 cm. and had a primary LTC/S. Subsequently, she had a missed Ab/D and C. This pregnancy, her BP since the first trimester has been 110 to 120/70 to 80. 2 weeks ago it was 130/85, and today she came in due to feeling poorly--non-specifically, a bit of pressure behind one eye, and increased leg swelling. She denies RUQ pain or scotomata. Her BP at work/somewhere was 142/92. BTW, a repeat section is planned at 39 weeks. Her, the two readings are 145/95 and 150/90 about an hour later. She has only gained 2 pounds in 2 weeks, has a bit of edema, and no proteinura. She is not hyperreflexic. What is your next step? Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
