The data from the Brtisih AFP studies WHEN 2 MS-aFP were done yieled 4% incidence of abn and if first abn, then repeated the incidnece of abn reduced by 50%.

Of that 2% (20/1000) 2/1000 ended up being either IUFD or IUGR with preterm delivery.

Very elevated values deserve an evaluation since 10% will have adverse outcome. What has never been shown by evidence based data is how do you follow the population and does it make a difference! ????
