48 yo P2 (or 3?) woman with perineal neruoma excised for dyspareunia in January. Since then she has been massaging estrace (17-beta-estradiol) cream into the introitus for vaginal integrity. Symptoms much improved (without the spicific site pain of a neuroma) but reports feeling like it tears. Exam - with minimal stretch put on the introitus she does indeed develop a tear in the surface epithelium over the perineal body. What would you suggest to apply to the perineal skin to enhance stretch?
