Mulders & Dieben, Fertil Steril, 2001;75:865-70 showed that you maintain follicular suppression by ultrasound and E2/Progesteron levels up to 35 days with one Nuvaring. So you can use them once a month, hence 12 a year

Actually you need 13 NuvaRings (there are 13 sets of 4 weeks in a year). Still a definite difference in the copays. If I am doing the continuous with pills I tell the patient to be sure to go for the mail in "3 month supply" option. Then she can get the 4 packs for 12 weeks for the 3 month co-pay. As medical / continuous therapy.
