Diuretics recommended as first line therapy, followed by ß-blockers if there are no asthma issues. All ß-blockers are approved by the AAP for use in breastfeeding, but metoprolol or labetolol are preferred. If ß-blockers contraindicated, probably the next choice would be hydralazine. Avoid ACE-I's during the first month. Neonates are very sensitive. After the first month, captopril preferred if ACE-I used.
