ACOG forms

My problem with the ACOG forms is they are SO SO incredibly inclusive - that it is very easy to not do one (or more) part of it -- if you don't have it 100% filled out and notations about the responses - and the chart is ever subpoenaed -- then it seems to me that you'd be out on a limb for not paying attention to every single box on the form.

What do others think?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
