WPW in labor

I have a 25 yo lady at 39 wks admitted with PROM. Several years ago, during a routine physical that included an EKG she was diagnosed with Wolff Parkinson White. She was subsequently seen by cardiology and they opted not to ablate her as she had no symptoms. She has continued to be asymptomatic during her pregnancy and other than her previous workup hasn't had anything else done for workup.

I can't say that I've seen WPW during labor. (I vaguely remember it being something I saw on the medicine service during med school) I would assume that her risk would be of developing another arrhythmia during labor. Does anyone know what that risk might be? Would you do cardiac monitoring during labor? Should she be at a level 1 center without a functioning ICU?

Thanks,

Glen

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
