Proctor should be from outside group if possible and should be done until the Proctor is ready to sign off or declare incompetence of individual involved.

Ron,

I think what you've outlined seems like more than enough given the proven track record of the doc you describe.

The thornier issue, which we just happened to be dealing with in Medical Exec committee today, revolves around what to do with newly minted docs fresh out of residency. As in what procedures require direct observation of ___ cases succesfully performed of each type before signing off that proctoring has been satisfied.

As an example, we have have three cases so far with a new gyn who finished residency last June - has had two failed laparoscopies (couldn't get scope into abdomen, piddled around for almost 45 minutes, trying first closed entry, then abortive attempt at Hasson open entry, never called for help) and also had a return to OR on postop day 1 after TVH for intraperitoneal bleeding, again failed to get a laparoscope in, had to open.

The senior partner of that group - in practice over 12 years and sporting a lower and lower volume of laparoscopic procedures each year for his entire tenure here - didn't pitch in to bail out the new grad for the return to OR case, despite being present in house and not otherwise tied up. Our OR always has many general surgeon laparoscopists around on a daily basis, as well as either myself or my partner, both avid endoscopists, in house or across the parking lot within 5 minutes walk.

What do y'all have as number of cases of proctoring for each type of procedure and by whom must the proctoring be done - can the proctoring be done by a practice partner or should it be an outsider?
