First I totally agree that this patient with abnormal bleeding would be at increased risk for Endometrial Ca. The persistent abnormal endometrial echo was the result of changes within the subendometrial myometrium. The thought is that this might represent a reactivation of areas of adenomyosis by the tamoxifen. Other pathologies can produce a similar sonographic appearance: Cystic atrophy with thin bands and Polyps. My personal experience suggests that the most common are polyps, which of course may contain a focus of cancer.
