largest vaginal birth I attended was just over 12 lbs. She went from 7cm to delivery in 2 contractions. Didn't even have time to put on gloves. She was flat on her back in bed....

One of the worst shoulder dystocias I had was a multip, thin, not-excessive weight gain, baby about 6 lbs, fast labor and very fast 2nd stage. The shoulders came out directly transverse.
