57461 ($376.70) Colposcopy of the cervix including upper/adjacent vagina; with loop electrode conization of the cervix

57460 ($341.46) Colposcopy of the cervix including upper/adjacent vagina; with loop electrode biopsy(s) of the cervix

57522 ($261.49) Conization of cervix, with or without fulguration, with or without dilation and curettage, with or without repair; loop electrode excision

The descriptions and Medicare rates are per Flash Code. As Gary said, concurrent colposcopy distinguishes the first two from the third and which is equivalent to a cold knife conization, the difference is that one simply uses the LEEP electrode to accomplish it.

The operator performing a 57460 uses the LEEP as a biopsy tool while performing colposcopy rather than a biopsy instrument, and does not excise the entire transformation zone, as one does for a cone biopsy.
