Robert,

Do you induce at 41 weeks regardless of Bishop score?

Thanks for asking, Charlie. No, if the cervix is ripe, then no problem. If they have an unripe cervix coming up on 41 weeks, and there is no other indication for induction - I explain to them about the increased risk of Cesarean section & induction with an unripe cervix, and the rationale for offering induction - if they request induction then they are admitted for ripening/induction. If they refuse induction at 41 weeks ( and this is RARE in my practice), we will continue the usual surveillance until 42 weeks, when I will once again repeat the aforementioned process.

In my environment, a IUFD after 41 weeks is my fault and tantamount to a disaster. I have reached the stage in my career where I don't give a darn about the number of sections for 'failure to progress'.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
