Thanks for the response...BTW, don't spend a lot of time feeling sorry you couldn't make the link work, at least waste no sorrow on me. I'm one of those cyber challenged types who thought for a long time cut and paste meant put a small section of a sticky note onto the monitor screen.

To Hank and Lynn and whoever else is interested: Unfortunately, my
copies of the contract I use are at my office, my scanner and I are at
home, and I am only working part-time now, so it will be a few days
before I can get it all together. In the meantime, try a google search
for "narcotic contract". The first link that comes up has a contract
that is even more comprehensive than the one I use (sorry, I tried to
make a link and couldn't make it work, it keeps taking me back to the
home page for the site---American College of Physician Executives).
During my search, I also encountered this interesting link to a posting
on a drug policy bulletin board:
http://www.drugpolicy.org/forums/index.cfm?fuseaction=read&forum=19&id=243&cid=7&mid=1049
Chronic narcotic use appears to be a huge can of worms that is only
getting bigger.
