To Hank and Lynn and whoever else is interested: Unfortunately, my copies of the contract I use are at my office, my scanner and I are at home, and I am only working part-time now, so it will be a few days before I can get it all together. In the meantime, try a google search for "narcotic contract". The first link that comes up has a contract that is even more comprehensive than the one I use (sorry, I tried to make a link and couldn't make it work, it keeps taking me back to the home page for the site---American College of Physician Executives). During my search, I also encountered this interesting link to a posting on a drug policy bulletin board:

Chronic narcotic use appears to be a huge can of worms that is only getting bigger.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
