Less is better when it come to drugs. When I first strarted Ob/Gyn 80% of my practise was ob and 20% Gyn. The local birth rate is on the decline and now my practise is 50:50 OB/GYN with lots of geriatric gyn. It is now commom pratcise to see several pts. 65+ in one day; I know I am in trouble when just after telling me they have no real medical problems they take out thier pill tackle box, you konw the boxes with at least 10 compartments and I have seen some with 20 compartments all filled with pills and ofcourse being a Doctor I must know what the pills are by just looking at them. Well its not uncommon to have a pt on 2-3 antihypertensives, 2 Nsaids, losec or zantec and it seems almost everyone is on the Latest seritonin reuptake inhibitor for depression. Well by the time I am finished with the history I almost always send them for an anaesthetic pre-op consult for possible unstable angina, severe arthrithis,type II diabetes ...... . In Canada if you are over 65 you pay very little for your drugs in most regions I wonder if this has something to do with the polypharmacy.
