Gestational age is not mentioned in Williams or in the 1998 FDA Public Health Advisory regarding vacuum extraction as a contraindication or even as a concern. In Modern Instrumental Delivery by O'Grady, He mentions that "the use of vacuum extraction in the delivery of preterm infants, except second twins, is not advised, due largely to concern over intracranial injury although some authors indicate no unusual complications from such applications." So where is the data that proper application of vacuum is dangerous to a preemie coming from, besides the med-mal literature? When I was in residency 75-79, we were taught to avoid vacuum and use forceps on preemies to "protect" their CNS, although I think even that has been shown to be an invalid recommendation. Ronald E. Mom can push baby out, Doctor can help with vacum, Doctor can help with forceps, Doctor can help with c-section, The possible complications are the same for all of the above. Yes the rates are different. You can not say one methode is better/safer than the others because they are used in different circumstances. There is no Randomized controlled trials comparing the four options, and there never will be.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
