Beta Strep Gr B Prophylaxis

At Bronson Hospital in Kalamzoo, Michigan, the protocal for beta strep proplylaxis is identical with the CDC and ACOG recommendations with one exception. If the patient has EVER been cultured positive for beta strep Group B, even in a past pregnancy, she receives prophylaxis. Even if multiple cultures are negative during the current pregnancy.

Has anybody out there had experience with similar protocols? I am worried that by giving so many patients ampicillin (our drug of choice) we might be treating some people unnecessarily, and possibly increasing the risk of producing resistant organisms in L&D or the NICU.

Any comments?
