We notify patients only if the Pap smear is abnormal. We notify patients only if Mammogram is abnormal We notify patients only if DEXA is abnormal We notify patients only if FOBT is abnormal We notify patients of normal or abnormal labs (blood work, biopsies, etc.)

In our system, no results are placed on the chart until the doc has signed off and dated it. We also have a place for the person who put it on the doc's desk to sign and date it and the person who puts it in the chart to sign and date it. This really promotes responsibility by all concerned. I don't sign off on a chart until I have contacted the patient if needed, so it won't accidentally be filed with my signature already on it.

You can very easily construct your own 'tickler file' on most any relational database. No need to pay someone to do it.
