Lynn: A tough situation, as you're torn between trying to grant the patient autonomy (her right to choose) and potentially upsetting your colleagues. In the long run, upsetting or alienating a colleague over a patient situation such as this is not good. The patient may be one for life, or move on, etc., but you still will want a good relationship with your call group. On the other hand, it is difficult to tell a patient you don't want her to be in your practice (well, I can think of exceptions, as we all can). FWIW: I would consider discussing this discretely with the doctor from whom she wants to switch, stressing that you are in a tough spot and didn't at all solicit the patient. I would also say that the easiest thing to tell the woman is that a switch isn't possible for relationship/political reasons. I would not necessarily tell the other doctor the patient's name, though he may figure it out. Alternatively, you might tell the patient that this is politically incorrect for you (I have used an example that I cannot favor a patient over my family, for instance), as patients may come and go, but you're "married" to your call group. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
