Interesting recent article by Frank Chervenak in the green journal about this very issue (elective CS). His recommendations from an ethical perspective:

1. Not justified to OFFER elective CS

2. OK to do it at patient request, after appropriate counseling, etc.

This is what I have been doing. He makes a very elegant argument from an ethical perspective.

In this case, I might consider offering as well, despite these recommendations.
