Wait a minute. "Ask the Patient?" We are supposed to be the professionals.
I would tell the patient that a double nuchal cord was found, tell her that
many babies are born with nuchal cords and even true knots without any
problems, but let her know that it could cause problems during or before
labor if it were to tighten -which is not predictable and not a high
percentage. I would tell her to watch fetal movement consistently and
report to L&D if any acute changes - increased or decreased. I would offer
NST 2x wkly until delivered. If there are severe variables in labor,
deliver by C/S. If she strongly requested a C/S I would most likely agree
to it, but not without a couple of weeks of negotiation and discussion.

Ask the patient. Offer a primary C-section or attempt a vaginal delivery. If
there are abnormalities on the FHR tracing in labor, then cut.

Para one @ 36 weeks has an ultrasound - all parameters normal vertex, was
breech previously. Has a double nuchal cord on ultrasound. Now what?
